Community Stroke Rehabilitation Team
A free home- and community-based rehabilitation service for adult stroke survivors, offering therapy, system navigation and support to regain independence.
ConnexOntario
A free, confidential support service providing information and referrals for mental health, addiction, and problem gambling services across Ontario.
Counselling Services Available Through Primary Care
If you belong to any of the following team-based primary care organizations, check with your clinic regarding their counselling services: Central Community Health Centre, East Elgin Family Health Team, Thames Valley Family Health Team, and West Elgin Community Health Centre (walk-in counselling available at West Elgin Community Health Centre on Thursdays from 1pm-4pm for anyone in the community). If you don't currently have access to a family physician or nurse practitioner, mental health services are available through the Elgin Community Health Hub.
Crohn's and Colitis Canada (London Chapter): Peer Support
A volunteer-led network offering connection, understanding, and shared experience for people living with Crohn's disease or ulcerative colitis, as well as their families and caregivers. Provides local peer support, education, and community engagement opportunities.
Cultural Resource
The Cultural Resource Program provides access to traditional teachings, ceremonies, and cultural activities, fostering a positive Indigenous identity and well-being for children, youth, and families.
Dale Brain Injury Services Peer Support Program
A free support program connecting individuals who have experienced an acquired brain injury with trained peer mentors. Participants meet one-on-one or in groups to share experiences, build coping strategies, and support each other's journeys.
Diabetes Education: West Elgin Community Health Centre
Our Diabetes Care Team offers individual and group education with flexible hours at multiple locations in Elgin County. All adults living with diabetes or prediabetes, and those at risk for diabetes are invited to access these free services.
Diabetes: The Group Lifestyle Balance Program
A 7-month program that provides education, encouragement, and the tools necessary to help individuals reach their healthy lifestyle goals. The program aims to help individuals achieve a balanced lifestyle through sustainable nutrition and physical activity strategies. In addition, this program has the potential to help individuals with prediabetes significantly reduce their risk of developing Type 2 Diabetes. Groups meet weekly for the first 3 months and then biweekly for the remainder of the program.
Dorothy Day Learning Centre
The Dorothy Day Learning Centre at N'Amerind Friendship Centre offers access to cultural resources, education, and social activities, promoting understanding of Indigenous history and traditions while fostering community engagement.
