The Elgin OHT is a group of local health, community, and social service organizations working together to improve how care is delivered in St. Thomas and Elgin County. We focus on building a more connected, people-centered health system.
Our team includes hospitals, primary care providers, mental health, substance use and addictions services, home and community care organizations, social support agencies, people with lived experience, and others. We collaborate to make the healthcare journey easier for everyone.
If you don't currently have a primary care provider, register with Health Care Connect, Ontario’s program that helps match people with a primary care provider in the Elgin region.
