Sweet Talk: Managing Your Blood Sugars
In this virtual group, a registered dietitian, a registered nurse/registered practical nurse, and/or a registered pharmacist discuss the importance of blood sugar control and associated risk factors. This group is intended for individuals who are at risk for or have pre-diabetes or newly diagnosed diabetes.
Telehomecare
Enables patients to become partners in their own care, right in their own homes using technology; help people living with conditions such as chronic obstructive pulmonary disease (COPD) achieve the best possible quality of life while learning to manage their condition, through remote monitoring and regular health coaching sessions in their own homes.
Thames Valley Family Health Team
Provides team-based primary care, chronic disease management, mental health supports, and access to allied health professionals to help individuals and families stay healthy and well.
Thames Valley Family Health Team (TVFHT)'s Group Programming
TVFHT offers many group and individual programs across a variety of topics: Best Weight, Dream On (Insomnia), Introduction to Mindfulness, and more
The Annex
A free, compassionate service offering addiction support groups, counselling, peer support, and referrals for mental-health, medical, dental, legal, financial and housing assistance.
The Family Central
Provides 24/7 emergency shelter and transitional housing, including overnight accommodations from November 1 to March 31. Services include meals, health care and medical aid, counselling and support, as well as systems navigation to help individuals access housing supports, Ontario Works, disability benefits, and other community services.
The Family Central's Food Security Program
Provides food support, including grocery hampers and emergency food assistance to individuals and families experiencing food insecurity in the community.
The Fanshawe College Institute of Indigenous Learning
The Fanshawe College Institute of Indigenous Learning provides a welcoming, culturally supportive space for First Nations, Métis, Inuit, and non-status Indigenous students. It offers academic advising, peer tutoring, cultural and spiritual programming, social activities, study spaces, and access to knowledgeable staff who support students' academic, personal, and cultural success throughout their educational journey.
The Four Counties Community Transit System
Operates a wheel-chair accessible bus available to the residents of the Four Counties area, including the counties of Elgin, Kent, Lambton and Middlesex. To reach dispatch or for more information, call the number. The cost to take the Four Counties Transit bus is $7 one way. Trips can be dispatched through the program contacts also.
